Medicare Advantage (Part C)

Since 1997· Insurance type

Depends on youExperts differ; the right answer depends on your situation.

Private health plans, approved and paid by Medicare, that replace Original Medicare and usually bundle drug coverage and extras.

How it works

You still pay the Part B premium, but the plan, usually an HMO or PPO, handles hospital and doctor coverage, often for no added premium and with dental, vision or gym perks. Trade-offs: provider networks, prior approval for many services, and a yearly in-network out-of-pocket cap of up to $9,250 in 2026. About 54% of eligible beneficiaries were in a plan in 2025. Main trap: returning to Original Medicare later may mean Medigap health questions. HICAP gives free, unbiased help choosing.

Why it matters for you

Depends on you: lower premiums and extras versus narrower networks and approval hurdles. The choice made at 65 can be hard to reverse later.

Rules, limits and sources (2026)

KFF enrollment analyses (about 34 million people, 54% of those eligible, in 2025); CMS plan data and star ratings; MedPAC's annual reports to Congress.

Watch out for

Whether Advantage plans deliver better care or mainly cost taxpayers more is debated; MedPAC estimated 2025 payments about 20% above traditional Medicare's.

Connections (2)

▸Medicare Advantage (Part C) is a private-plan route within Medicare (federal health insurance at 65+)Standard practice

How they connect

Medicare pays private insurers a monthly, risk-adjusted amount per enrollee to deliver all Part A and B benefits, usually with Part D. Enrollees stay in Medicare and still pay the Part B premium, but the plan's network, rules and copays replace Original Medicare's. Insurers use part of the payment to fund extras and lower premiums.

When it applies

Private Medicare contracts began in the 1980s; the 1997 Balanced Budget Act created Medicare+Choice, renamed Medicare Advantage by the 2003 Medicare Modernization Act.

How sure?

Social Security Act Part C (Sec. 1851 onward); CMS rate notices; KFF data show plans covering more than half of eligible enrollees since 2023.

Watch out for

Whether insurers' diagnosis coding inflates payments, and by how much, is disputed among insurers, MedPAC and federal auditors.

▸Medicare Advantage (Part C) competes with Medigap (Medicare Supplement) & CA birthday ruleDepends on you

How they connect

At 65 most people pick one path: Original Medicare plus Medigap and Part D (higher premiums, few bills, any doctor) or Medicare Advantage (low premiums, copays up to a yearly cap, networks). You can't combine them. Moving from Advantage to Medigap later usually means health questions, except under a 12-month trial right or special situations.

When it applies

The choice comes in the 6-month Medigap window at 65 and each October 15–December 7 open enrollment; HICAP counsels Californians free.

How sure?

Social Security Act Sec. 1882(s) on guaranteed issue and trial rights; CMS's 'Medicare & You' handbook compares the two paths.

Watch out for

Studies disagree on which path yields better health outcomes; some find sicker enrollees leave Advantage plans more often.
