In-Home Supportive Services (IHSS)
Since 1973· Government program
Law or rule (2026)Set by statute, regulation or the agency; numbers change yearly.
California's Medi-Cal program that pays caregivers, often family members, to help frail or disabled people keep living at home.
See it on the Personal Finance & Estate map →How it works
A county social worker visits, assesses need and authorizes monthly hours (up to 283 for the most impaired) for bathing, dressing, meals, housework, errands and some paramedical tasks. The recipient hires and directs the caregiver; the state runs payroll at county wage rates. Adult children can be paid caregivers, and spouses in limited cases. More than 700,000 Californians use it. Who it's for: Medi-Cal members who are 65+, blind or disabled. Main trap: hours are often fewer than families expect, but decisions can be appealed.
Why it matters for you
It turns home care, the setting most parents prefer, into a paid Medi-Cal benefit, and lets a relative earn wages for care they may already give.
Think of it like
Like a management contract the owner directs: Medi-Cal pays the staff, but the family picks and supervises them.
Rules, limits and sources (2026)
Created by state law in 1973 (Welfare and Institutions Code Sec. 12300 onward); the California Department of Social Services publishes monthly caseload and hours data.
Watch out for
Assessed hours vary by county and social worker, and state budget fights periodically propose trimming hours or caregiver overtime.

▸In-Home Supportive Services (IHSS) pays home caregivers through Medi-Cal (California's Medicaid)Law or rule (2026)
How they connect
IHSS is a Medi-Cal benefit, so a person must qualify for Medi-Cal, sometimes with a monthly share of cost, to get paid caregiver hours. Federal Medicaid dollars cover roughly half the cost, with the state and counties paying the rest. Counties assess need and enroll providers; the state pays them.
When it applies
Started in 1973 as a state program; folded into Medi-Cal's personal care services in 1993 and the federal Community First Choice option in 2011.
The rule
Welfare and Institutions Code Sec. 12300 onward; CDSS and DHCS budget documents show the federal, state and county cost split.
Watch out for
With costs rising as California ages, how to fund IHSS growth, and whether to cap hours or overtime, recurs in budget debates.
▸In-Home Supportive Services (IHSS) subsidizes the home option in Home care vs. assisted living vs. nursing home: costsLaw or rule (2026)
How they connect
Private home care in California often costs $35 or more an hour, roughly $6,500–7,500 a month at 44 hours a week (estimates vary). For Medi-Cal members, IHSS pays for an approved number of those hours, making staying home far cheaper than assisted living, which Medi-Cal covers only through limited waiver slots.
When it applies
IHSS runs in all 58 California counties; family members are common caregivers, and pay rates are set county by county.
The rule
CareScout (formerly Genworth) Cost of Care surveys for private rates; CDSS data on IHSS hours and county wage rates.
Watch out for
Whether paid family caregiving saves Medi-Cal money by delaying nursing-home moves is plausible but hard to measure precisely.
